The 2026 NYC School Survey is open now until April 1st! A paper copy was sent home with students. The PPAS survey raffle drawings are continuing – two family names will be selected and contacted directly.
Complete the survey on NYCSchoolSurvey.org, where it is available in 13 languages. You’ll need your students’s nine-digit student identification number, or OSIS, to begin filling out your survey online.
You can find the OSIS number on your student’s report card, or school ID card. Once on the Survey page, you must enter a lowercase “f”, followed by your child’s OSIS to access the survey. Note that the “f” must be lowercase.
