The 2026 NYC School Survey deadline has been extended to April 17th! A paper copy was sent home with students.
Complete the survey on NYCSchoolSurvey.org, where it is available in 13 languages. You’ll need your students’s nine-digit student identification number, or OSIS, to begin filling out your survey online.
You can find the OSIS number on your student’s report card, or school ID card. Once on the Survey page, you must enter a lowercase “f”, followed by your child’s OSIS to access the survey. Note that the “f” must be lowercase.
